CME speaker and Abstract presenter forms
LTC Robert Harrington (robert.dennis.harrington@us.army.mil) is the 2008 Program chairman for SOMA 2008. Should you want to make a presentation, contact Dr Harrington before September 17 LTC Harrington has the authority of the Board of Directors to accept or refuse presentations, based on the theme of the Conference and the contents as applied to the conference.
You will receive confirmation from Dr Harrington by September 19th that you have been chosen a speaker and/or a panel presenter at the 2008 Conference. After you receive your confirmation from LTC Harrington, visit this web site and complete the “Disclosure Form” “Presentation Worksheet” and “Bio-Data Form.” This is crucial that the forms are received by TRUE for the attendees to receive CME or CEU credits for your presentation. If the two forms are not received in the TRUE office by October 17th the date the program goes to the printers, your presentation will not be approved for CME’s or CEU. The forms ask you to fax them directly to USUSH, but when you hit the “send” button they will be sent directly to TRUE who will process the forms.
